trans-1,2-cyclohexaandicarbonzuur wordt gebruikt als reagens bij de synthese van enantiomeerzuivere (α-cyaanalkyl) (tetrahydropyrido [4,3-b] indolecarbonyl) cyclohexaancarboxamiden en zijn analogen als selectieve cathepsine K-remmers voor de behandeling van osteoartritis. Ook gebruikt als reagens bij de synthese van nieuwe cyclopentaandicarboxamide-natriumkanaalblokkers als een mogelijke behandeling voor chronische pijn.
